Job Purpose:
The Care Manager role manages new client care consultations, care plans,client introductions, quality assurance visits, and client risk assessments. They also carry out supervisions, deal with various day-to-day issues and regularly update and maintain records. This role requires strong relationships with the HSE, public health nurses and other multi-disciplinary teams.
Job Specific Responsibilities:
• Conduct consultations and prepare care plans.
• Conduct risk assessments in the home and mitigate any risks
• Monitor, mediate and manage both client and Caregiver concerns and issues.
• Carry out Quality Assurance (QA) visits in client homes, developing and reviewing care plans for clients and conducting service enquiries and care consultations as needed.
• Through the QA visits, ensure client care plan is in line with their care needs, following up with family and healthcare professionals as appropriate.
• Ensuring the care being delivered in client's homes is meeting HCCI, ISO & Franchise Standards, local policies and the requirements of the Health Service Executive (HSE)
• Ensure Caregiver Supervisions to be kept up to date.
• Mentoring and supporting a large team of Caregivers to carry out their roles effectively.
• Develop and maintain client relationships and client family/ caregiver relationships. - Build networks and enhance existing relationships with health care professionals and multi-disciplinary teams in hospitals and various community settings
• Report and consult with senior management on clinical issues as appropriate.
• To participate in clinical audit as identified by the organization.
• Assist with other duties as delegated by the Management Team.

Education | Experience | Requirements:
• Minimum NFQ; QQI Level 6, Social, Healthcare degree preferred.
• 3 years' experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience.
