Purpose of the Post

Successful candidates will lead a co-ordinated approach to the management of all services withinAdult Physical &Sensory Disability Services and ensure an integrated approach to the delivery ofServices. He/she will be required to work closely with the Disability Service Manager in relationto priorities, quality assurance, co-ordination of voluntary sector, equity and service evaluation.

The HSE has created six new health regions. Each region is responsible for providing bothhospital and community care for the people in that area. Bringing community health services andhospitals together means we can take a more patient-centred approach to healthcare.HSE Dublin North East provides health and social care to North Dublin, Louth, Meath, Monaghanand most areas of Cavan.Community Healthcare Services are delivered through the HSE and its funded agencies topeople in local communities, as close as possible to people's homes. Community HealthcareServices focus on keeping you well so that you can continue to live at home or close to homeOctober 2025through our health promotion, disease screening, diagnosis, treatment, and rehabilitationprograms. You can refer yourself to most Community Healthcare Services or through your GP,public health nurse, community mental health team, etc. Services include - Primary Care, SocialCare - Older Persons, Social Care - Disabilities, Mental Health, Health and Wellbeing, Quality,Safety and Service Improvement.HSE Dublin North East Region includes the following hospitals; Beaumont Hospital Cavan General Hospital Connolly Hospital Louth County Hospital National Orthopaedic Hospital Cappagh Monaghan General Hospital Mater Misericordiae University Hospital Our Ladys Hospital Navan Our Lady of Lourdes Hospital Rotunda Hospital

The Adult Physical & Sensory Team deliver services to adults who meet the criteria for the AdultPhysical and Sensory Disability team. It is the aim of the APSD Team to provide a holistic, personcentred community based service to all service users.In Cavan/Monaghan the Adult Physical and Sensory Disability team provides a multidisciplinaryservice to adults aged between 18 and 65years, who present with Static and progressive neurological conditions Traumatic and non-traumatic brain injury Other physically disabling Neurological conditions

The team consists of a Social Worker Team Lead, Acquired Brain Injury Ireland case manager,Clinical Nurse Specialist, Regional Community Worker for MS Ireland, physiotherapists, ClinicalPsychologist, Speech & Language therapist, Social Worker and Occupational Therapist andclerical support. The service is provided within a holistic team based approach. This service isdelivered in collaboration within all services in the Cavan/Monaghan IHA.The multidisciplinary team provide assessment, diagnostic and therapeutic intervention andsupport to service users and their families. Onward referral is facilitated as appropriate. Supportis also offered in relation to applications for home support, day service, vocational and educationand residential options.

The team maintains close links with Acute Services, Rehabilitation Services and Voluntarybodies working in Cavan/Monaghan.Adult Physical and Sensory Services (APSD) are delivered through centre, community and homebased settings. This can be on an individual or group basis.

The Social work team leader will not have an operational line management role for social workers.

Qualifications and/or Experience

1. Professional Qualifications, Experience, etc

(a) Eligible applicants will be those who on the closing date for the competition:

(i) Must be registered in the social Work Register maintained by the Social Work Registration Board at CORU.

And

(ii) Maintain live annual registration on the Social Workers Register maintained by the Social Workers Registration Board at CORU

And

(iii) Must have the requisite knowledge and ability (including a high standard of suitability, professional and managerial ability) for the proper discharge of the duties of the office.

And

(iv) Have three years relevant post qualification experience.

Health

A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would

indicate a reasonable prospect of ability to render regular and efficient service.

Character

Each candidate for and any person holding the office must be of good character.

Post Specific Requirements

Demonstrate your depth and breadth of Social Work experience in the area of Multi-Disciplinary Community Work, Safeguarding

Vulnerable clients and must have experience and knowledge of legislative framework and have experience of preparing and implement necessary requirement for legal application and court appearance.
