Clinical Nurse Specialist - Heart Failure / (potentially leading to) Candidate Advanced Nurse Practitioner (cANP) / Advanced Nurse Practitioner (ANP) - Cardiology

Salary €62,699.00 - €79,221.00 dependent on relevant experience
The Purpose of This Role:
The Heart Failure Clinical Nurse Specialist (CNS), Candidate Advanced Nurse Practitioner (cANP) or Advanced Nurse Practitioner (ANP) will provide specialist nursing care to patients with heart failure across inpatient and outpatient settings.
The post holder will play a key role in the development and delivery of a comprehensive Heart Failure Service by coordinating patient care, supporting early diagnosis, optimising evidence-based medical therapy, facilitating timely investigations and interventions, and promoting continuity of care throughout the patient journey.
Working collaboratively with the Consultant Cardiologist and multidisciplinary team, the successful candidate will contribute to improved clinical outcomes, reduced hospital admissions and readmissions, enhanced patient experience and the ongoing development of nurse-led heart failure service

The post holder will:
• Provide specialist assessment and management of patients with suspected or confirmed heart failure.
• Undertake comprehensive clinical assessment including history taking, physical examination and interpretation of relevant investigations appropriate to the role.
• Coordinate outpatients and inpatient heart failure consultations and reviews.
• Participate in consultant-led outpatient heart failure clinics.
• Develop and deliver nurse-led heart failure clinics (where appropriate to grade and scope of practice).
• Optimise guideline-directed medical therapy in accordance with agreed clinical protocols.
• Monitor patients during medication titration and provide ongoing clinical review.
• Identify patients suitable for ambulatory management and facilitate early supported discharge where appropriate.
• Coordinate post-discharge follow-up and ensure continuity of care.
• Act as a point of contact for patients requiring specialist heart failure advice.
• Promote patient self-management through education regarding medication adherence, symptom recognition, diet, fluid management and lifestyle modification.
• Escalate complex or deteriorating patients promptly to the supervising Consultant Cardiologist.
Essential Requirements:
Be registered as a Registered General Nurse (RGN) with the Nursing and Midwifery Board of Ireland (NMBI).
• Have a minimum of 5 years post-registration clinical experience.
• Have a minimum of 3 years recent experience in cardiology, heart failure service, acute medicine, cardiac diagnostics.
• Demonstrate evidence of continuing professional development in cardiology heart failure nursing.
• Possess excellent clinical assessment and communication skills.
• Demonstrate competency in ECG interpretation and cardiology patient assessment.
• Demonstrate competence in venepuncture and phlebotomy.
