Case Manager, Housing First, Mid West
Location: Limerick
Contract: Full Time, 1 Year Fixed Term
The purpose of this role is to work as a part of Housing First Case Management Service to deliver a ‘Housing First’ approach to individuals experiencing long term homelessness in Limerick City.
What you will do:
Work in line with the principles and fidelity of the Housing First model, promoting client choice, independence and self-determination.
Provide a safe, welcoming and supportive service for individuals who are homeless, at risk of homelessness or living in temporary accommodation.
Build and maintain trusting, professional relationships with clients, supporting them to achieve greater stability and sustain independent living.
Complete assessments, care plans and support plans using Housing First case management systems and outcome measurement tools.
Support clients to access and maintain suitable housing, welfare rights, entitlements and move-on accommodation opportunities.
Assist tenants to establish and sustain successful tenancies, including developing life skills, managing rent payments, reporting repairs and being a good neighbour.
Maintain and develop strong links with social housing providers, private landlords, homelessness services and other accommodation providers.
Accompany and advocate for clients at appointments with healthcare, counselling, legal, social and other support services where appropriate.
Work collaboratively as part of a multidisciplinary team and in partnership with statutory, voluntary and community agencies.
Provide advice, information and guidance to prevent homelessness and support clients in accessing education, training, employment and community-based opportunities.
Manage an active caseload, ensuring accurate and timely case notes, assessments, daily records and database updates using Focus Ireland systems and PASS.
Attend supervision sessions, team meetings, handovers, training and external meetings, while contributing to service development and best practice standards.
Work flexibly to meet service needs, maintain health and safety standards, support volunteers or students when required, and undertake additional duties as reasonably assigned.
What you will need:
A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology, etc and/or be a CORU registered Social Care Worker
Experience of working with customers are or have been homeless
Experience of working with customers identified as having a dual diagnosis
Experience of team working
Report writing abilities
Experience of working and liaising with other agencies
Knowledge and understanding of homelessness and issues that affect people who are or have been homeless (Desirable)
Full driver’s license (Desirable)
