Case Manager, Family Homeless Action Team, Dublin
Location: Dublin
Contract: Full Time, Permanent
The purpose of this role is provide onsite support to families in emergency accommodation. To assess, support and advocate for families with the view to moving them from homelessness into stable accommodation.
What you will do:
Manage an allocated caseload and adhere to care and case management processes and protocols.
Carry out assessments and reviews and develop and implement support plans with families referred to the service.
To work within a client-centred, psychologically informed environment and trauma informed approach, encouraging autonomy and independence.
Maintain accurate records and statistics in accordance with Focus Ireland policy. To work with and under the supervision and direction of the line management within the project.
Participate in the evaluation and development of the service.
Make contact with and establish links as appropriate with local services and relevant community groups and to work closely with emergency accommodation and housing providers .
Be familiar with and comply with all relevant Focus Ireland policy e.g. Child Protection Policy, Confidentiality Policy, Customer Complaints Policy, Health and Safety Policy, Personnel Policy.
Be vigilant to any Health, Safety and Welfare risks in the workplace and bring any concerns to the attention of your Line Manager or Health Safety representative.
Participate in relevant training and development courses as agreed with your Manager/Team Leader.
Undertake such other duties as might be reasonably assigned from time to time in consultation with the Manager.
What you will need:
A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology etc and/or be a CORU registered Social Care Worker.
Proven experience working in a social care setting.
Full clean driver’s license and use of car. (Desirable)
Experience of dealing with child protection and welfare responsibilities and responding appropriately to any concerns. (Desirable)
Experience in carrying out assessments, including risk assessments. (Desirable)
