To provide a direct Medical Social Work service to patients, their partners and families as assigned by the Principal Medical Social Worker
To keep the Principal Medical Social Worker informed on matters pertaining to casework practice particularly in relation to: Critical cases, Critical Department/Hospital Matters, Medical Social work team developments;
To provide counselling, information and support as appropriate to hospital patients as per assigned caseload;
To keep and maintain such records and statistics as are part of the structure of the Medical Social Work Department;
To provide case management support for complex discharge planning;
